Full Facial Balancing Model Program

Thank you for your interest in becoming a model for Soul Aesthetics Full Facial Balancing Program with Nurse Injector Hayley.

This program allows selected patients to receive customized injectable treatments at a reduced rate following Hayley’s 5 day Intensive Fundamental training course. Because we value expertise and safety, Hayley will be showcasing her skills from her training to our PA Kelle Christianson and will be using this opportunity to get before and after photos of her results.

All treatments are supervised and performed using the FDA-approved Allergan Juvederm Collection of fillers and Botox.

Model Pricing

Because this will be full facial balancing, expect to pay a minimum of $1600. However, model pricing will be based on treatment plan and quantity of product needed and may be up to $3500. If you are selected, we will contact you with an estimated amount.

Requirements

To apply, you must:

  • Be 25+
  • Be in good health
  • Prefer filler naive patients or no filler in the last year
  • Have no contraindications to injectables
  • Be comfortable with before/after photos
  • Be available during designated model days which will be Monday-Friday during working hours.
  • Agree to using your photos/videos for all marketing purposes
  • Be following Soultulsa and hayleyrn_soultulsa

As a model, you understand:

  • You will be required to fill out a health history form and consents for policies and treatment.
  • Results may vary
  • Treatments are educational in nature and you may have multiple people in the room
  • Touch-ups are not guaranteed
  • You will be required to do a 2-3 week follow up after your treatment
  • Posting on your own social media and tagging us is welcome and encouraged (not required)
  • In order to be granted a spot and be scheduled, you will have to put down a $250 nonfundable deposit that will be applied to your balance at check out.

If you are contacted and do not reply within 24 hours, your spot will be given to the next person in line.

Photo Upload

You need to upload 4 photos of your face. You must have your hair pulled back in a pony tail and/or headband. These will turn out better if you stand against a wall and have someone take them. Include slightly above of the top of the head to midchest. The better your pictures, the better chance of selection.

1. Face Forward NO SMILE
2. Turn 45 degrees NO SMILE
3. Turn 45 degrees the opposite direction NO SMILE
4. Turn facing to the side (lateral) NO SMILE
